Does the conceal power work when someone sees you on the astral? Ie do they get the penalty when assensing a person who is trying to hide.

No. It is a physical power, not an astral one.

Since the power is physical, the spirit must be on the physical plane to cast it right?
Does that mean the spirit has to conceal itself? And does that count as one of the targets?

As it is for SR5, and by my understanding, Yes To All.

It doesn't have to conceal itself; you can have it conceal you all and leave it in the car. But if you want to bring it, and want it to stay hidden, it's a good idea to have it conceal itself, yes.

It's also quite possible that a spirit can materialize, use the Conceal power, then return to the astral while maintaining the power. There is no requirement that a magician must be on the same plane as a sustained spell is cast in order to sustain it, after all.

As a shaman in my SRM group discovered (RIP, Papa Remy), mages can see you just fine.

Which is why I said "Yes" to that question, as if you've got more services to use and are likely to need spiritual help you're gonna have that spirit close by. Otherwise it'd be like this infiltrator story I recall reading somewhere, where a guy made sure to take every possible precaution to avoid detection physically & wirelessly, except for the RFID tag in his underwear that an attentive spider noticed.
