We are in the midst of Thamel, so there are a lot of people around - but few who care if someone around dies or screams or does anything else that isn't directly remoted to money. That is, to their money. This also depicture the milieu of the area. This is the bad part of the former tourist area. This is were the bloody streetdocs and the mercenaries go, where you can buy organs and second-hand augmentations discreetly and where you can purchase child soldiers and girls without questions asked. This is very cops only go undercover - and only if they must.
Where or not you parked your van nearby is your decision. If you did, you must be aware that it's difficult to go anywhere from her by car since the streets are extremely crowded and you have to force your way through. Also, you should be aware that this is probably the area in Nepal where the most criminal energy meets the most skill - so your car is not safer than you are.
It's still daytime, so the real dark creatures are still in their caves. But in an area like this, that's not saying a lot because no-one is ashamed to follow their business in open daylight.
And just a hint. You can either assume that the doc doesn't have his valueable gear in his clinic - or that he has it heavily guarded and the guards are nearby.
