Okay, so I have several actions I need to take, the first of which is to Levitate the big galoot off of his spear, then stabilize and diagnose him, then first aid him, then heal him.  I am pretty sure at some point in there, probably during the Heal spell, that the spear needs to come out, but until then what little I know about wound trauma is that it needs to be left alone.  The problem will be getting him out of here, so I think Devon will Levitate him up and turn him over, and stabilize him here, then try to get him out of here if he thinks he has enough time to get him back to the clinic before the 'magic hour' is over.

Devon would know the right way to do things so if my inexpert process is wrong I will edit.

Well, couple of points:

a) Since he hadn't overflowed his CM, he didn't need to be stabilized. Kind of moot at this point, but...figured I'd throw it out there.

b) I'm not a doctor/EMT, so just make it sound good. :)

Incidentally, Chrona's rolls to see how long it would take him to heal were off by...quite a bit, actually. Healing tests are Body x 2 minus situational modifiers, but ignoring condition modifiers. So if Armless were to rest up at his street-level home, he'd have 10 dice per test; if Devon just gets him back to the clinic, that goes up to 12. And, of course, there's whatever bonus dice he gets from medicine. Kind of hard to keep a troll down, I guess.

Well, that is another problem, I have no way of knowing or even assuming you are Awakened.  That could also be a problem, because if I work on you without that modifier I could impact you Magic rating, at least that used to be the case.

Siegfried to the rescue.  I can tell you're awakened, and can heal you magically.  One thing I'm not clear on is whether the spear has been pulled out of you yet.  If not, it needs to be pulled out right before I cast the heal spell, otherwise it either won't work, or will close your wound around the spear.

Also, I think I need to know is essence for the purpose of casting the spell- reduced essence reduces my dice pool for heal, right?

Really? That's a harsh penalty...<checks rules> Nope, don't see anything like that. It's just up to us to remember to apply all appropriate modifiers. (I forgot the Awakened one above; incidentally, Armless just has an armored vest and a lined coat on, so the full armor penalty to first aid isn't an issue.)

And at four meters per six seconds (which, interestingly enough, is Flatline's normal walk rate), you're looking at getting back to the clinic in about ten minutes. This all went down close to your own backyard, as it were.

That is only when you are worried about insurance, I can wait and do the healing in a sterilized environment.  Also, don't sweat it, I forgot the astral bit, no worries I will know you are awakened.  I got this you will be fine.
